The service is funded by Regional Integration Fund and managed by West Glamorgan Regional Partnership.
The purpose of the project is to provide a seamless, long-term, wrap-around community support service for people with dementia and their carers across our region. The project will not only signpost people with dementia and their carers to relevant local support, but also ensure that once someone enters the service, they are provided with long-term, wrap-around assistance to help people address all of the challenges that the condition presents.
